艾特雅®为循环往复接受外用药但仍治疗控制不佳、亟需系统治疗的中重度斑块状银屑病患者提供了一种创新治疗选择。
中国北京,2026年8月27日 —— 强生公司今日宣布,白介素23(IL-23)受体拮抗剂艾特雅®(盐酸伊可白滞素片)已正式获得中国国家药品监督管理局批准,用于适合系统治疗或光疗的中重度斑块状银屑病成人患者和12岁及以上且体重40公斤及以上的儿童患者1。
艾特雅®是首个且目前唯一能够精准阻断IL-23受体的靶向口服肽类药物2。国家药品监督管理局药品审评中心于2025年11月授予其优先审评资格,以支持该创新药物的加速获批3。近期,国际银屑病理事会发布的最新指南明确界定了从外用药物治疗转为系统治疗的标准4,这一更新不仅反映了全球银屑病诊疗格局的演变,更为艾特雅®等创新一线系统治疗方案的应用提供了重要背景依据。
临床证据摘要
在总计纳入2500名患者的四项Ⅲ期临床研究中,艾特雅®达到所有主要疗效终点,且展现出良好的安全性特征5,6,7。本次获批基于ICONIC临床开发项目完备的循证证据,该项目同步开展成人与青少年人群研究,评估艾特雅®针对头皮、生殖器等高影响部位皮损的疗效,并开展了两项与活性对照药物的头对头试验。
在头对头优效性研究中,约70%的患者在第16周达到皮损完全或几乎完全清除(IGA 0/1),55%的患者达到银屑病面积和严重程度指数改善≥90%(PASI 90)应答,显著优于安慰剂和氘可来昔替尼(deucravacitinib)8,a,b。在ICONIC-LEAD研究中,第16周时,85.1%的中国入组患者达到IGA 0/1,71.6%达到PASI 90应答;第24周时,约70%的中国入组患者实现皮损完全清除(IGA 0)9。 截至第16周,艾特雅®治疗组患者不良反应发生率与安慰剂组差值不超过1.1%,且截至第52周,未识别出新的安全信号10。
中重度斑块状银屑病存在未被满足的临床需求

银屑病影响着中国超过840万患者13,不仅影响身体舒适度,也严重降低生活质量,当皮损出现在暴露或敏感部位时尤为显著。对于许多中重度银屑病患者而言,靶向系统治疗至关重要14。这与国际银屑病理事会的指南相一致,即如果连续两轮、每轮4周的外用药物规范治疗仍未带来显著改善,应及时切换至系统治疗4。

关于 ICONIC 临床开发项目
关键性 III 期 ICONIC 临床开发项目包括四项针对中重度斑块状银屑病患者开展的艾特雅®III 期研究,这些研究为此次获批提供了支持。

- ICONIC-ADVANCE 1(NCT06143878)和 ICONIC-ADVANCE 2(NCT06220604)是两项随机对照试验(RCT),旨在评估艾特雅®对比安慰剂及氘可来昔替尼(deucravacitinib),在成人中重度斑块状银屑病患者中的疗效和安全性。19,20
关于斑块状银屑病
斑块状银屑病(PsO)是一种慢性免疫介导性疾病,导致皮肤细胞过度增生,从而形成炎症性、鳞屑的斑块,可伴有瘙痒或疼痛21。据估计,中国约有840万患者11,全球则有超过1.25亿人患有此病22。 近四分之一的斑块状银屑病患者属于中度至重度病例20。斑块通常表现为隆起的皮损,表面覆盖着银白色的死皮细胞或鳞屑。在肤色较浅的患者中,斑块可能呈红色;而在肤色较深的患者中,则可能呈现紫色、灰色或深棕色。斑块可出现在身体的任何部位,但最常见于头皮、膝部、肘部和躯干23。患有斑块状银屑病会带来诸多挑战,其影响不仅限于患者的身体健康,还涉及情绪健康、人际关系以及应对生活压力等方面24。若银屑病出现在身体显眼部位或敏感皮肤区域(如头皮、手、足和生殖器),对生活质量的负面影响会进一步加剧。19,25
关于艾特雅®(盐酸伊可白滞素片)
艾特雅®(盐酸伊可白滞素片)是首个且唯一一款旨在精确阻断IL-23受体的靶向口服肽类药物,而IL-23受体是中重度斑块状银屑病炎症反应的核心机制2, 26,27。艾特雅®与IL-23受体具有高亲和力,并在人体T细胞中表现出对IL-23信号传导的强效抑制作用28。 这些发现的临床意义尚不明确。
艾特雅®已获美国批准,现亦在中国获批,用于治疗适合接受系统治疗或光疗的中重度斑块状银屑病成人患者和12岁及以上、体重≥40 kg 的青少年患者。服用艾特雅® 的患者每日一次服用一粒,于晨起后、进食前 30 分钟温水送服1。
